We provide coordinated services for total health care, including access to specialty care, behavioral health services and hospital care. Our care team includes physicians, medical assistants, patient care associates, registered nurses and outreach coordinators.
Between visits, we can provide chronic disease management support, including medication education and lifestyle coaching, to help you manage your health at home. If you are hospitalized, we will call you after discharge to answer any questions and ensure proper self-care. If you have received care outside of our practices, our staff will gladly assist you in transferring your records. At every Drexel Medicine office, we use electronic health records to ensure that our patients receive accurate, coordinated health care.
