HIV Case Management

Managing a chronic condition like HIV can be overwhelming, especially when you are facing challenges that make it difficult to stay healthy. The Partnership understands this. It is why we have medical case managers (MCMs) to help patients get more of what they may need to be well.

Our medical case management services are open to patients who receive primary medical care for HIV/AIDS at the Partnership.

HIV Medical Case Managers Can Help You:

  • Access food, housing and transportation
  • Schedule appointments with medical specialists
  • Get and keep medical insurance
  • Get legal help for needs like wills, custody, benefit appeals, etc.
  • Get and replace IDs, social security and insurance cards and other necessary documents

Additionally, medical case managers can help you access the following:

  • Drug/alcohol treatment referrals
  • HIV information
  • Housing referrals
  • Legal services
  • Medical equipment
  • Mental health treatment referrals
  • Prenatal care
  • Social security benefits
  • Special pharmaceutical benefits
  • Support groups
