HIV Case Management
Managing a chronic condition like HIV can be overwhelming, especially when you are facing challenges that make it difficult to stay healthy. The Partnership understands this. It is why we have a team of social workers and other social services staff to help patients get more of what they may need to be well.
Our social services are open to patients who receive primary medical care for HIV/AIDS at the Partnership.
Our HIV Social Services Team Can Help You:
- Certify for Ryan White services
- Support with HIV education and medication adherence
- Get and keep medical insurance and specialty medication coverage
- Schedule appointments with medical specialists
- Connect to dental services
- Assist with transportation to medical appointments
- Access emergency financial assistance programs for things like utility shut-off notices, past due rent, and initial move-in costs
- Connect to a variety of food and nutritional resources
- Get legal help for needs like wills, custody, benefit appeals, etc.
- Connect to and navigate prenatal care for pregnant patients
- Assess for ongoing and long-term medical case management services
- Additionally, our team can help patients connect to the following:
- Behavioral health treatment referrals
- Housing resources
- Legal services
- Medical equipment
- Social security benefits
- Support groups
More HIV Social Services
Eligibility Specialist – The Partnership’s eligibility specialist helps our patients with accessing medical insurance (Medicaid, Medicare, private insurance and health insurance subsidies [Pennie], as well as Pennsylvania's Special Pharmaceutical Benefits Program (SPBP). The Eligibility Specialist also certifies those eligible for Ryan White services.
Perinatal Program – Our perinatal medical case manager works with pregnant people to reduce the risk of transmission of HIV to the fetus. They provide support and assistance in accessing resources during pregnancy and ensuring pre-natal and post-natal medical visits are attended.
Contact HIV Social Services at the Partnership
You can access HIV Social Services and be assessed for varying levels of medical case management at the Partnership by asking to speak with the clinic case manager on-call during your appointment. You can also call the Partnership at 215.762.2530 (Option #4) and leave a message asking to discuss medical case management options. You must be a patient at the Partnership clinic to receive social services.
