HIV Case Management

The Partnership Comprehensive Care Practice (the Partnership) offers medical case management services to those receiving primary care for HIV/AIDS. Medical case managers (MCMs) are available to help with access to food, housing and transportation, as well as scheduling medical specialist appointments and getting and keeping medical insurance.

Medical case managers may also assist with obtaining legal help for issues like wills, custody, deeds and benefit appeals. MCMs also help with getting and replacing IDs, Social Security cards, insurance cards and other documentation. Additionally, they can help access:

  • Drug/alcohol treatment referrals
  • Food resources
  • HIV information
  • Housing referrals
  • Legal services
  • Medical equipment
  • Medical insurance
  • Mental health treatment referrals
  • Prenatal care
  • Social Security benefits
  • Special pharmaceutical benefits
  • Support groups
  • Transportation

Specialized Case Management Services

Eligibility Specialist – The Partnership’s eligibility specialist assists with accessing medical insurance, Social Security benefits, or Pennsylvania's Special Pharmaceutical Benefits Program (SPBP). The eligibility specialist also certifies eligible clients for Ryan White services.

Prenatal program – Our perinatal medical case manager provides pregnant patients living with HIV with the help and support they need to ensure a healthy pregnancy and delivery, as well as postpartum care for both parent and child. They provide assistance in accessing resources during pregnancy and support at prenatal and postnatal medical visits.

Contact us – Medical case management services at the Partnership can be accessed by requesting to speak with the medical case manager on call during a medical appointment or by calling the Partnership at 215.762.2530 (Option #4). Please leave a message asking to be referred for medical case management services. To receive medical case management services, you must be a current patient at the Partnership.
